Pathological Analysis of 2865 Cases of Intraductal Papillary Lesions by Ductoscopy

Abstract


The aim of this study is to explore the pathological analysis of 2865 cases of
intraductal papillary lesions by ductoscopy and to discuss the features of ductoscopicappearance of malignant cases. We reviewed 2865 papillary lesions diagnosed using ductoscopy from April 23, 2013 to October 31st, 2017 who were evaluated by using surgical excision biopsy and pathology. Data concerning patient age, discharge and the details of endoscopic appearance were analyzed according to the pathological results. A total of 2715 patients complained of pathological nipple discharge (PND) were collected. The postoperative diagnosis in 2290 patients (79.9%) was benign and 260 patients (9.08%) was ADH. A total of 315 (11.0%) cases were found to be malignant. In our study, on univariate analysis, patients of with spontaneous and bloody discharge were more likely to suffer from malignant lesions. On logistic regression analysis, bloody nipple discharge, morphology, and a broad lesion base revealed by ductoscopy showed a statistically significant correlation with malignancy (p<0.05,respectively). The endoscopic features of bloody discharge, morphology, and a broad lesion base are independent risk factors for malignancy and represent new criteria for the diagnosis of patients with PND.
