The Effect of Polyhexamethylene Biguanide Complex Irrigation in Polyacrylamide Hydrogel Removal Operation of Breast

Abstract

Polyacrylamide Hydrogel (PAHG) has been commonly used as medical filling
agent in the treatment of a variety of soft tissue defect disease. However, when PAHG breaks down into monomers in human body, it could cause a series of complications. In China, Polyacrylamide Hydrogel removal operations are necessary for a large number of patients who had received breast augmented operation with PAHG and were suffering with its complications. Polyhexamethylene biguanide (PHMB) complex has been proved to be effective in wound with inflammation. The aim of this study was to evaluate effect of
Polyhexanide-betaine irrigation in Polyacrylamide Hydrogel removal operation of breast. We undertook a prospective study of 32 patients who underwent bilateral PAHG removal operation of breasts between June 2016 and December 2017. Intraoperative irrigation with Polyhexanide-betaine solution was taken in one side (PHMB group), and with saline in the other side (saline group). Indicators of recovery after the operation in two groups werecollected and used to evaluate the effect of PHMB. In our analysis of 32 patients treated with
PHMB irrigation, the volume of drainage was significantly reduced in PHMB group compared with saline group (P<0.05). The period with the drainage tube has been obviously shortened in PHMB group compared with saline group (P<0.05). And The VAS score in the PHMB group was significantly decreased ompared with the saline group 48 hours after the operation. In conclusion, the Polyhexanide-betaine solution has a positive effect on Polyacrylamide Hydrogel removal operation of breast. It could help to reduce the total
amount of drainage and shorten the recovery period.
